연구 목적: 본 연구는 임플란트 식립 후 10 년 이상 추적 관찰된 임플란트 인접치와 동일 환자의 반대측 대조치아를 비교하여, NCCL, 치아 우식(Implant-facing proximal caries (IFPC) 및 Non-Implant-facing caries (NIFC)), 근관치료, 간접수복치료, 치아파절, 발거 등 7 개 임상지표 및 전체합병증(≥ 1 event)의 누적 발생률 및 발생 시점을 분석함으로써, 임플란트가 인접 자연치에 미치는 장기 영향을 평가하고자 하였다. 연구 대상 및 방법: 2010 년 1 월 1 일부터 2015 년 12 월 31 일까지 강원대학교치과병원에서 구치부 임플란트 치료를 받은 환자 1,391 명(임플란트 2,013 개)중, 1 년부터 10 년까지 전 추적기간에 걸쳐 의무기록 및 방사선사진이 가용한 총 238 명(임플란트 264 개)을 대상으로 후향적으로 검토하였다. 각 환자에서 임플란트에 인접한 자연치(실험군)와 동일 악궁 반대측에 위치하는 동일 종류의 자연치(대조군)를 대응시켜 7개 임상지표 및 전체 합병증의 환자 내 비교를 시행하였다. 임플란트 인접치는 추가로 근심치아와 원심치아로 구분하여 분석하였다. 누적 발생률 비교는 McNemar test, 발생 시점 비교는 Mann-Whitney U test로 분석하였다. 결과: 임플란트 단위 분석(n = 264)에서 NCCL은 실험군(13.6%)에서 대조군(7.6%)에 비해 통계적으로 유의하게 더 많이 발생하였으며, 전체 합병증도 실험군(34.5%)에서 대조군(26.5%)에 비해 많이 발생하였다. 치아별로 분석한 결과 주로 원심치아보다 근심치아에서 NCCL과 전체 합병증이 많이 발생하였다. 결론: 임플란트 인접치는 반대측 대조치아에 비해 NCCL 및 전체 합병증 발생 빈도가 유의하게 증가하였고, 이러한 차이는 근심치아에서 두드러졌다. 임플란트 식립 후 인접 자연치, 특히 근심측 소구치 부위에 대한 면밀한 장기 추적 관찰이 필요하다. □ 핵심주제어 치아우식증, 치과 임플란트, 후향적 연구, 치아 발거, 치아 파절, 치아 마모

Purpose: This study aimed to evaluate the long-term effects of dental implants on adjacent natural teeth by comparing implant-adjacent teeth followed for more than 10 years after implant placement with the contralateral teeth of the same patients. The cumulative incidence and time-to-event of seven clinical indicators—non-carious cervical lesions (NCCL), dental caries (implant-facing proximal caries [IFPC] and non-implant-facing caries [NIFC]), endodontic treatment, indirect prosthetic treatment, tooth fracture, and extraction—as well as any complications (≥ 1 event) were analyzed. Materials and Methods: Among 1,391 patients (2,013 implants) who received posterior implant treatment at Kangwon National University Dental Hospital between January 1, 2010, and December 31, 2015, a total of 238 patients (264 implants) with complete medical records and radiographs available throughout the entire follow-up period from 1 to 10 years were retrospectively reviewed. For each patient, the natural tooth adjacent to the implant (study group) was paired with the same type of natural tooth located on the contralateral side of the same arch (control group), and within-patient comparisons of the seven clinical indicators and any complications (≥ 1 event) were performed. Implant-adjacent teeth were further classified into mesial and distal teeth for analysis. The cumulative incidence was compared using the McNemar test, and the time-to-event was compared using the Mann–Whitney U test. Result: In the implant-unit analysis (n = 264), NCCL occurred significantly more frequently in the study group (13.6%) than in the control group (7.6%), and any complications were also more frequent in the study group (34.5%) than in the control group (26.5%). Tooth-level analysis revealed that NCCL and any complications occurred predominantly in mesial teeth rather than distal teeth. Conclusion: Implant-adjacent teeth showed a significantly higher cumulative incidence of NCCL and any complications compared with the contralateral teeth, and this difference was particularly pronounced in mesial teeth.
