Effect of facemask therapy on the maxillary posterior structure: comparison of skeletal and tooth-borne anchorage

Objectives: To investigate changes in the maxillary posterior structure after a facemask therapy using the cone-beam computed tomography (CBCT) images, and to compare the differences between when using the skeletal anchorage and the tooth-borne anchorage. Materials and Methods: This retrospective study included 9 patients who used the miniplates as an anchorage for the maxillary protraction (Miniplate/FM group, 9.5 ± 1.4 years old [mean ± SD]), and 11 patients who were treated using the facemask and the rapid maxillary expander (RME/FM group, 9.2 ± 1.4 years old [mean ± SD]). Treatment changes on the maxillary posterior structures were evaluated using the CBCT images at pre-treatment (T1), and after facemask therapy (T2). The changes of measurements were compared within each group and between two groups. Results: The Miniplate/FM group showed greater advancement of the maxilla and midface than the RME/FM group. The amount of advancement of the pterygomaxillary suture of the Miniplate/FM group was about three times of those of the RME/FM group. The amount of advancement of the pterygomaxillary suture was about 50% of the advancement of A point in the Miniplate/FM group, and about 25% in the RME/FM group. Posterior part and superior part of the maxilla increased in the transverse dimension in the Miniplate/FM group. Conclusions: Greater forward movement of the pterygomaxillary suture in the Miniplate/FM group led to larger advancement of maxilla and midface than in the RME/FM group.

연구 목적: 본 연구의 목적은 상악 전방견인치료에 의한 상악 후방구조물의 변화를 cone-beam computed tomography (CBCT) 영상을 통해 평가하고, 치성 고정원과 골격성 고정원 간 차이가 있는지를 비교하는 것이었다. 연구 방법: 미니플레이트를 이용하여 상악 전방견인치료를 받은 9 명의 Miniplate/FM 군 (9.5 ± 1.4 세 [mean ± SD])과 11명의 급속 상악 확장장치를 이용하여 상악 전방견인치료를 받은 RME/FM 군 (9.2 ± 1.4 세 [mean ± SD])을 선정하였다. 치료 전, 후 상악 후방구조물의 변화를 CBCT를 이용하여 평가하고 측정값의 변화를 통계적으로 군내, 군간 비교하였다. 연구 결과: Miniplate/FM 군은 상악과 중안면의 더 큰 전방이동을 보였다. 두 군 모두에서 익돌상악봉합의 유의한 전방이동이 관찰되었고, Miniplate/FM 군은 RME/FM 군보다 약 3배 더 많이 이동하였다. Miniplate/FM 군에서 익돌상악봉합의 전방이동 양은 A point 전방이동 양의 약 50%정도였고, RME/FM 군은 25% 정도였다. Miniplate/FM 군은 상악 확장을 하지 않았음에도 불구하고, 상악의 후방부와 상방부에서 폭경의 증가가 관찰되었다. 결 론: 골성 고정원을 이용한 상악 전방견인치료시, 급속 상악 확장장치와 같은 치성 고정원을 이용할 때보다 익돌상악봉합에서 더 많은 골침착과 전방이동을 야기하였다.
