Dental Hygiene Intervention Classification Based on Dental Hygiene Process of Care

Objective: This study aimed to conceptualize the dental hygiene intervention performed by dental hygienists based on the dental hygiene process of care, which is the unique task of dental hygienists, and to systematize the dental hygiene practice by dental hygienists by organizing specific activities to provide dental hygiene interventions. Methods: The dental hygiene intervention classification was based on the qualitative content analysis method proposed by Elo and Kyngas. Dental hygiene interventions were derived by developing a classification matrix according to the clinical dental hygiene practice guidelines in Korea. In addition, by repeatedly reading dental hygiene textbooks, studies, and reports, meaningful statements about the research topic and dental hygiene interventions were discovered, coded, and organized into a dental hygiene practice. The contents of the primary dental hygiene intervention classification were refined after review by three internal experts. The final classification of dental hygiene interventions was derived through an expert Delphi survey conducted twice with 15 professors in charge of clinical dental hygiene in the department of dental hygiene nationwide. In the Delphi survey, the content validity, clarity, homogeneity, and mutual exclusion were evaluated. Results: As a result of the first and second expert Delphi surveys, the content validity ratio for all dental hygiene interventions, definitions, and practices was ≥0.60, and the content validity index was ≥0.80. The degree of agreement was a minimum of 0.80 and a maximum of 1.00. In the first survey, the degree of convergence was 0.50 in some items, and as a result of modifying the contents according to the opinions of the experts, the degree of convergence was improved to 1.00 in the second survey. Finally, 38 dental hygiene interventions and 451 dental hygiene practices were derived. As a result of checking dental hygiene interventions that correspond to each stage of the dental hygiene process of care, 8 interventions corresponded to dental hygiene assessments, 1 to dental hygiene diagnosis, 6 to dental hygiene plans, 19 to dental hygiene practices, 3 to dental hygiene evaluations, and 1 to documentation. Moreover, 21 of 38 dental hygiene interventions were named with the terms “examination,” “education,” and “management.” Conclusion: The dental hygiene intervention was conceptualized through the dental hygiene intervention classification, and the essence of the dental hygienist was confirmed. A dental hygienist is an oral health expert who helps in preventing oral diseases and promoting oral health by providing evidence-based comprehensive preventive management through interaction with the clients and is a primary care worker who can contribute to health promotion. In the future, dental hygienists are expected to be actively involved in the primary care system and dental clinical sites and contribute to health promotion through practical discussions for this purpose.
