목적: 전치부 개방교합을 나타내는 성인 환자에서 교정치료로 인한 교합 개선 후 전두동의 크기 변화의 차이에 대해 알아보고자 하였다. 연구방법: 개방 교합 환자 중 절충 치료를 한 17명의 환자(비수술군, 24.5±6.1세)와 악교정 수술을 동반한 교정 치료를 받은 13명의 환자(수술군, 21.1±3.6세), 그리고 초진 시 정상적인 전치부 교합을 가진 환자 17명(대조군, 19.8±3.3세)을 이번 후향적 연구의 대상자로 선정하였다. 치료 전(T1)과 치료 후(T2)의 CBCT에서 ITK-snap을 이용하여 전두동의 부피를 계측하였다. 그룹 내(the Wilcoxon signed rank test), 그룹 간(the Mann-Whitney U test with Bonferroni correction) 전두동의 부피의 차이를 통계적 방법을 통해 비교하였다. 결과: 상악동의 부피는 비수술군에서 치료 후 유의하게 감소하였다. 그러나, 대조군과 수술군에서는 상악동의 부피는 치료 전후 유의한 차이가 나타나지 않았다. 결론: 성장이 완료된 성인 환자에서 전치부 교합이 개선 후 전두동 하방부의 opacity의 증가와 함께 전두동 크기의 감소가 나타날 수 있다.

Aim: To investigate the changes of the frontal sinus after non-surgical or surgical orthodontic treatment for anterior open bite in adult patients. Materials and Methods: Samples for this retrospective study consisted of cone beam computed tomography (CBCT) images taken from 30 patients who received orthodontic treatment to correct anterior open bite. Seventeen patients (non-surgical group, 24.5±6.1 years old) were treated with orthodontic treatment, and 13 patients (surgical group, 21.1±3.6 years old) received orthodontic treatment with orthognathic surgery. Another seventeen patients (control group, 19.8±3.3 years old) with minor crowding and normal anterior overbite and overjet were selected as a control group. The volume of the frontal sinus was measured using the CBCT images before (T1) and after treatment (T2). Volumetric changes were compared within groups (the Wilcoxon signed rank test) and between groups (the Mann-Whitney U test with Bonferroni correction). Results: Volume of frontal sinus significantly decreased at T2 in non-surgical group compared to control group. But no significant differences in volume were found in surgical and control group after treatment. Conclusions: Size of the frontal sinus may be reduced slightly with increases in opacity on the inferior part of sinus after improvement of anterior occlusion in open bite adult patients.
