목적: 골격성 III급 아동에서 급속구개확장장치 (RME) 를 이용한 확장 여부에 따른 상악골 전방견인술 후 자연두부위의 변화를 평가하였다. 연구방법: RME 확장을 동반한 상악골 전방견인 치료를 받은 18명의 III급 부정교합 환자들 (RME/FM group, 평균 연령: 8.3세±2.0) 과 RME 확장 없이 상악골 전방견인 치료를 받은 16명의 III급 부정교합 환자들 (non-RME/FM group, 평균 연령: 8.4세±1.4) 이 이 후향적 연구의 대상자로 선정되었다. 치료 전 (T1), 치료 직후 (T2) 의 측모 두부계측방사선사진과 T1, T2, 그리고 약 치료 1.5년 후 (T3) 의 자연두부위 상태에서 촬영된 측모 사진을 이용하여 두부계측방사선사진의 계측치 및 자연두부위의 변화를 두 그룹에서 그룹 내 및 그룹 간 비교를 하였다. 결과: T1에서 두부계측방사선사진의 계측치는 두 그룹 간에 유의한 차이가 없었다. T1-T2 시기 동안, 그리고 T2-T3 시기 동안에 canthus-tragus line과 진수평선과의 각도로 계측한 자연두부위는 RME/FM 그룹에서 각각 0.5° 와 -0.7°였고, non-RME/FM group에서 각각 0° 와 1.0° 였다. 치료 기간 (T1-T2) 과 관찰 기간 (T2-T3) 동안 자연두부위는 그룹 간 및 그룹 내에서 통계적으로 유의한 차이가 없었다. 결론: 본 연구 결과에 따르면 III급 부정교합 아동에서 시행하는 상악골 전방견인 치료는 RME 확장 여부에 관계없이 자연두부위의 변화를 초래하지 않는 것으로 보이며, 이 결과는 1.5년 간의 관찰기간 동안 유지되었다.

Aim: To investigate whether changes in natural head position (NHP) vary depending on maxillary protraction with or without expansion of rapid maxillary expander (RME) in skeletal Class III children. Materials and Methods: Eighteen Class III patients who received maxillary protraction therapy with RME expansion (RME/FM group, mean age: 8.3 ± 2.0 years old) and sixteen Class III patients who were treated by maxillary protraction therapy without RME expansion (non-RME/FM group, mean age: 8.4 ± 1.4 years old) were selected for this retrospective study. Lateral cephalometric radiographs at pre-treatment (T1) and post-treatment (T2), and lateral facial photographs in NHP at T1, T2, and post-treatment (T3) were collected for this study. Changes in the cephalometric measurements and NHP were compared between groups. Results: The cephalometric measurements between two groups in T1 show that there were no significant differences between groups. NHP changes which were measured by the angle between canthus-tragus line and true horizontal line during T1-T2 and T2-T3 were 0.5° and -0.7° in RME/FM group, respectively, and those in non-RME/FM group were 0° and 1.0°, respectively. During the treatment period (T1-T2) and observation period (T2-T3), there were no statistically significant differences in NHP changes in either between groups or within groups. Conclusions: The results of this study suggest that natural head position in Class III children did not change significantly after maxillary protraction therapy regardless of maxillary expansion, and it maintained during a 1.5-year observation period.
