연구 목적 : 본 연구의 목적은 하악 매복 제3대구치의 발치 후 발치와의 치유에 collagen plug의 투입이 어떠한 영향을 미치는 지를 확인하고자 하는 것이다. 연구 방법 : 심하게 매복된 하악 제3대구치 발치 후 발치와에 collagen plug를 넣은 경우를 실험군으로 하고, 발치 후 아무것도 넣지 않은 경우를 대조군으로 하였다. 대상 환자들의 챠트 및 수술 전, 수술 후 3개월, 수술 후 6개월의 방사선사진에 대한 계측으로 후향적 연구를 통하여 실험군과 대조군간 치조골이 재생된 양의 변화를 술 후 3개월 및 6개월에 각각 비교 분석 후 그 결과를 평가하였다. 연구결과 : 술 후 3개월에 골재생량 (Bone gain)은 발치 후 발치와에 아무것도 넣지 않은 경우에는 1.47 ± 0.50 mm였으며, 발치 후 collagen plug를 넣은 경우에는 1.50 ± 0.43 mm로, 평균 골재생량은 발치 후 collagen plug를 넣은 경우가 더 높았지만, 두 군사이에 통계적으로 유의한 차이는 없었다 (P>0.05). 술 후 6개월에서 실험군과 대조군의 골재생량은 발치 후 발치와에 아무것도 넣지 않은 경우는 2.44 ± 2.49 mm였으며, 발치 후 collagen plug를 넣은 경우에는 5.04 ± 1.35 mm이었다. 평균 골재생량은 발치 후 collagen plug를 넣은 경우가 더 높았고, 두 군 사이에 통계적으로 유의한 차이가 있었다 (P<0.001). 결 론 : 본 연구에서는 발치와에 collagen plug을 적용한 경우 수술 후 6개월의 소견에서 아무런 이식재도 적용하지 않은 군에 비하여 우수한 골재생을 보여주었다. 따라서 심하게 매복된 하악 제3대구치 발치 후 collagen plug를 적용하는 것은 발치 후 골재생에 도움이 되는 것으로 보인다.

Purpose: The purpose of this study was to evaluate the effect of collagen plug application into the extraction socket after the mandibular third molar surgery. Patients and methods: After the extraction of the impacted mandibular third molar, the experimental group received the collagen plug and the control group did not receive any graft. The amount of bony defect was measured at the time of surgery, 3, and 6 months after extraction, respectively. The difference between groups was compared. Results: There was no statistically significant difference in bone gain between groups at 3 months postoperatively (P> 0.05). At 6 months postoperatively, bone gain was significantly higher in the experimental group than that in the control group (P <0.001). Conclusion: The collagen plug application into the extraction socket reduced the amount of the bony defect after the third molar surgery compared to the untreated control. Therefore, application a collagen plug after extraction of the impacted mandibular third molar appears to be helpful for bone regeneration after third molar extraction.
