구강악안면영역의 골 결손을 수복하기 위하여 임상적으로 많은 종류의 골 이식 재료가 사용되고 있다. 4-hexylresorcinol(4-HR)은 염증 반응의 주요 요소인 nuclear factor kappa B pathway를 저해하는 것으로 알려져 있는 물질로, 최근에는 이식재의 흡수 및 손상과 관련 있는 foreign body giant cells의 형성 억제 및 골파괴세포 형성 억제에도 관여하는 것으로 밝혀졌다. 본 연구에서는4-HR을 처리한 이종골(돼지뼈)을 인위적으로 형성한 쥐의 두개골 결손부에 적용한 후 방사선학적 및 조직학적 분석을 통하여 4HR을 처리한 실험군에서 대조군과 비교하여 골형성 효과가 촉진되고 염증 반응이 유의하게 감소된 것을 확인하였다. 또한 osteoblast-like cell을 이용한 세포 실험을 통하여 4HR이 Tumor necrosis factor-α 을 이용하여 인위적으로 유발한 염증을 감소시키는데 효과가 있음을 확인하였다. 결론적으로 본연구에서 4HR을 처리한 이종골 이식재료가 골 형성 효과를 촉진하고 염증반응을 감소 시키는 효과가 있음이 입증되어 새로운 이식재로의 가능성을 확인하였다.

Bone defects in the oral and maxillofacial region often cause problems with the installation of dental implants and their survival rate. Various bone graft materials have been used to overcome these problems. 4-hexylresorcinol (4HR) inhibits the nuclear factor kappa B pathway, which is a key component of the inflammatory response, and also inhibits the formation of foreign body giant cells, which damage graft materials by phagocytosis. In addition, 4HR has anti-osteoclastogenic effects. In this study, using radiological and histological analysis, we found that porcine bone particles with incorporated 4HR used as a bone graft material decreased the inflammatory response and increased osteogenesis in a rat calvarial defect model. We also found an anti-inflammatory effect of 4HR in an inflammatory environment induced by a proinflammatory cytokine (Tumor necrosis factor-α) in vitro. We suggest that porcine bone particles with incorporated 4HR have a dual effect; increase of bone regeneration and inhibition of inflammation. These properties would be also be useful for regeneration of other tissues.
