Objective: We hypothesized that tooth wear of the maxillary central incisor is differs depending on the treatment modality in Class III malocclusion patients with anterior crossbite. The present study aimed to compare the amount of tooth wear of the patients treated maxillary protraction followed by fixed appliance with those of the patients treated by only camouflage treatment. Materials and Methods: The samples consisted of the dental casts from 58 patients who received orthodontic treatment to correct anterior crossbite. The samples were divided into three groups according to the type of treatment and the age. Group I consisted of 21 subjects (7 males and 14 females; mean age: 9.8 years; range: 7.3 - 12.4 years) with 2-phase treatment (bonded rapid maxillary expander and facemask followed by fixed appliance). Group IIa consisted of 15 adolescent subjects (8 males and 7 females; mean age: 13.5 years; range: 10.3 - 17.3 years) with camouflage treatment. Group IIb consisted of 22 adult subjects (13 males and 9 females; mean age: 24.5 years; range: 18.7 - 49.1 years) with camouflage treatment. The maxillary dental casts taken before (T1) and after (T2) orthodontic treatment were scanned, and 3D images of the maxillary central incisors were constructed. The maxillary central incisors at T1 were superimposed with those at T2 with the best-fit method. Tooth wear were quantitatively calculated, then it was compared between both groups. Results: The amount of tooth wear was significantly lesser in the Group I subjects compared with Group IIa and Group IIb subjects. And there was no significant difference between adolescent group (Group IIa) and adult group (Group IIb) who received camouflage treatment. The Spearman correlation analysis revealed no significant correlations between tooth wear and age, treatment duration, or craniofacial morphology in Group I and Group II, respectively. Conclusions: Our findings suggest that early treatment of skeletal Class III malocclusion with anterior crossbite has an additional benefit which prevents the loss of tooth structure compare to the camouflage treatment.

본 연구의 목적은 전치부 반대교합을 동반한 골격성 III급 부정교합 환자에서 2 단계 교정치료(접착형 상악급속확대장치와 상악전방견인장치를 이용한 악정형 치료 후 고정식 장치를 통한 교정치료를 시행하는 방법)를 시행하는 경우와 고정식 교정장치 만으로 절충치료를 시행하는 경우, 치료 방법에 따른 상악 중절치의 절단면 치아 마모량의 차이를 비교하는 것이다. 연구 대상은 전치부 반대교합 개선을 위해 교정치료를 받은 골격성 III급 부정교합 환자 58명의 석고 모형이며, 치료방법과 연령에 따라 그룹 I (2단계 교정치료, 21명), 그룹 IIa (절충치료, 18세 이하, 15명), 그룹 IIb (절충치료, 18세 이상, 22명)로 분류하였다. 총 108쌍의 치료 전후 상악 중절치에서 절단면 마모량을 측정하였다. 교정 치료 전(T1)과 치료 후(T2)의 상악 석고 모형을 스캔하여 상악 중절치의 3차원 이미지를 생성하였고, Rapidform XOR3®(INUS Technology Inc, Seoul, South Korea) 상에서 상악 중절치의 교정 치료 전후 부피 변화를 측정하였다. 연구 결과 그룹 I의 상악 중절치 마모량이 그룹 IIa와 그룹 IIb에 비해 통계적으로 유의하게 작은 것으로 나타났으며, 절충 치료를 시행한 그룹 IIa와 그룹 IIb에서는 유의한 차이가 관찰되지 않았다. 각 그룹 내에서 성별에 따른 치아마모도의 차이는 관찰되지 않았으며 상관분석 결과 그룹 I과 그룹 II에서 치아마모도와 연령, ANB, FMA, 치료기간 사이에 유의한 상관성이 관찰되지 않았다. 본 연구에서는 3차원 역공학 기술을 이용하여 치아의 부피 차이를 측정함으로써 치아 마모도를 정량적으로 평가하였고, 전치부 반대교합을 동반한 골격성 III급 부정교합 환자에서 2단계 교정치료를 시행하는 것이 절충치료를 시행하는 것보다 절단면 치아 마모량을 줄이는 데 도움이 되는 것으로 나타났다.
