Purpose The purpose of this study was to evaluate the effects of implant shape and bone preparation on primary stability of the implants using resonance frequency analysis(RFA). Methods Sixty bovine rib blocks were used for this study. To simulate type II bone quality, cortical bones of 30 rib blocks were reduced to 1 mm of thickness. The cortical bones of the other 30 blocks were totally removed and the blocks were treated with 20% glacial acetic acid for 1 hour, and they served as type IV bone. Each rib block received 3 straight-screw type and 3 tapered-screw type implant fixtures (ShinhungImplant M vertical implants and tapered implants). Final drilling were done at 3 different depths for each implant types ; 1 mm underpreparation, standard preparation, and 1 mm overpreparation. Immediately after fixture insertion, implant stability quotient(ISQ) was measured for each implant. Results Regardless of the bone type, the ISQ values of the straight-screw type and tapered-screw type implants were not significantly different(P> 0.05). Depth of bone preparation had no significant effect on the ISQ value of straight-screw type implants(P> 0.05). For the tapered-screw type implants, underpreparation significantly increased the ISQ value(P< 0.05), whereas overpreparation significantly decreased the ISQ value(P< 0.05). Conclusion Bone quality seemed to have a prevailing effect over implant shape on primary stability. The primary stability of the tapered implants might be enhanced by delicate surgical techniques.
