Objectives: This study was performed on the pit and fissure sealing program that consists of the largest part of the currently ongoing school-based oral health programs to understand the problems of actual application of the guideline of the Ministry of Health and Welfare and to suggest an improvement means by surveying the current management situation of the pit and sealing programs carrying out at the health centers and subcenters in the Gangwon province and the eruptive period of first permanent molars of the first grade students at the Joo Moon primary school in Kangnung City in the year of 2005. Methods: The survey was conducted in 76 sites where a public health dentist is stationed, among 18 city and town health centers and subcenters and 96 health center branches in the Gang Won province. The person in charge of the pit and fissure sealing program was interviewed by telephone inquiry as well as e-mail and fax, and the response was received from 56 sties (73.7%), and the frequency analysis of the entire questionnaire questions was performed. The pattern of the eruptive period of the first molars was assessed by following-up of the first grade students at the Joo Moon primary school in the year of 2005 for 1 year and a half. Results: The management situation of the pit and fissure sealing program of the health centers and subcenters in the Gang Won province was surveyed, and it was found that the proportion of health centers (subcenters) managing the program according to the detailed guideline described in the 2006 guideline of the pit and fissure sealing program of the Ministry of Health and Welfare was low. As problems, retention, check-up, resealing time, unrealistic aim, cooperation of involved departments and organizations, a vague standard for resealing, not detailed and unclear program guideline, etc. were suggested. In regard to the pattern of the eruptive period of first permanent molars, the complete exposure of the occlusal surface by the first term of the second grade of primary school and the functional occlusion state were approximately 90%. Conclusion: To maximize the currently ongoing pit and fissure sealing program, based on the questionnaire and the pattern of the eruptive period of the first permanent molars, the 2006 guideline of the pit and fissure sealing program of the Ministry of Health and Welfare can be improved as follows. 1. The establishment of whole target figure should be transformed the subject number into the tooth one needed pit and fissure sealing. The guideline should be changed limiting within three teeth for each person into extending to four teeth(the first permanent molars) and the method of assessment should be transformed into the assessment of a target figure based on a required quantity of the pit and fissure sealing that can do a quality management and assessment. 2. The subject of the fit and fissure sealing program should be become from the period of the complete exposure of occulusal surface, and examining the pattern of the eruptive period of the first permanent molars, even until the first term of the second grade, only 90 % of the entire first permanent molar become the potential subject of fit and fissure sealing, and thus the grades for the concentrated management of fit and fissure sealing program should be established at least from the first grade to the second grade. 3. The caries of the first permanent molars should be prevented by increasing the number of subjects through the restriction of the subject teeth of the pit and fissure sealing program from 16 teeth including premolar teeth to 4 first permanent molars or managing the subjects repeatedly. 4. Considering the diversity of the eruption time and the time of exposure of occlusal surface among individuals and tooth types as well as the period of the retention of pit and fissure sealing, the currently ongoing protocol of the promotion method of the pit and fissure sealing program that is based on the class and number should be changed to the examination and management method of 3 months unit on the first and second grade students of primary schools. 5. To maintain the quality of treatment, through strengthening the resealing standard as the discoloration of the margin of sealing material, the ill-fit margin detected by an explorer and sealing material with air bubbles in addition to partial fracture and complete falling off of the sealing materials the quality follow-up management of the first permanent first molar as well as the understanding and improvement of the problems generated during treatments should be allowed. 6. Regarding the pattern of the record of the first permanent molar, the necessary treatments in addition to the tooth condition after examination should be recorded and thus the tooth formed a cavity that caries is on going currently should be performed the early sealing program simultaneously, and the tooth required sealing should be carried out the pit and fissure sealing program at an appropriate time, and the tooth required resealing should be performed the early resealing.
